Demi Moore has entered rehab.

The actress, who suffered a seizure after smoking an “incense-like substance” on January 23rd, began receiving treatment late last week.

I wonder if the visit from Bruce Willis and Ashton Kutcher was some sort of intervention?

“Demi’s life is in compete crisis. She has spiraled since her split from Ashton,” says a source of Moore, who also began chasing after daughter Rumer’s 24-year-old pal Zac Efron. “She is turning 50 in November and has no idea who she is or what her life should be.”

Demi is being treated at Cirque Lodge in Sundance, Utah, for an eating disorder and addiction issues. “She’s on total lockdown and only talking to a small group of people.”

Known for its individualized approach to addiction treatment, the super-posh Cirque Lodge has also helped Lindsay Lohan, Mary-Kate Olsen and Eva Mendes overcome their addictions.
